Transitions of Care RN Spanish Bilingual

Job Summary

A company is looking for a Transitions of Care RN - Spanish Bi-lingual.

Key Responsibilities:

• Provide compassionate care to diverse patients and their families
• Educate and triage individuals experiencing acute transitions of care
• Collaborate with interdisciplinary team to ensure continuity of care

Required Qualifications:

• Fluent in Spanish
• Hold an active, unencumbered, compact RN license
• Experience in caring for adult and pediatric patients with chronic illnesses
• Utilization Management or Case Management experience preferred
• Previous Transitions of Care experience is a plus
