Transitions of Care Supervisor

Job Summary

Leading a team of TOC Coordinators, the full-time remote Transitions of Care Supervisor will manage patient transitions from hospital to home, ensuring they receive necessary resources and support while also overseeing team performance and program development.

Key responsibilities

• Provide direct patient care for approximately 30% of the time while managing the TOC Coordinators' performance and adherence to key performance indicators
• Collaborate with leadership to develop and enhance the transitions of care program and participate in cross-functional meetings
• Assist in recruiting, onboarding, and training new TOC Coordinators, while also implementing process improvements and new workflows

Required qualifications

• MA Certification (active or expired) preferred; significant MA experience may be considered in lieu of certification
• At least 2 years of direct lead or supervisory experience in a virtual setting
• Experience in healthcare administrative or office roles for 2+ years
• Proficiency in technology and adapting to evolving platforms
• Organizational skills with a strong attention to detail
