Transplant Care Scheduler

Job Summary

A company is looking for a Transplant Care Scheduler (Remote).

Key Responsibilities

• Assist registered nurses with scheduling and care coordination for patients in the Transplant Care Navigation program
• Document all activities and update patient medical records while ensuring effective communication with patients and care partners
• Confirm patient appointments, reschedule missed appointments, and assist in obtaining test results

Required Qualifications and Education

• Associate's degree required
• 1 - 2 years of related experience, with at least 1 year in a clinical setting preferred
• Scheduling training and EPIC proficiency preferred
• Prior remote employee experience strongly preferred
• Ability to operate basic office equipment and maintain a HIPAA-compliant workspace
