Triage Registered Nurse (RN) - MN Licensed

Job Summary

A company is looking for a Triage Registered Nurse (RN).

Key Responsibilities

• Receives calls related to patient care and instructs patients/caregivers on appropriate interventions
• Assesses patient needs and determines the necessity for nursing visits based on EMR and telephone interactions
• Communicates with physicians and interdisciplinary teams to coordinate patient care and maintain continuity

Required Qualifications

• Graduate of an accredited school of nursing
• One to two years of recent acute care experience in an institutional setting
• One to two years of recent experience in home care preferred
• RN license for the states served or a compact license required
• Bachelor's degree preferred, with hospice care experience also preferred
