TX LA TN Licensed Nurse Practitioner

Job Summary

Providing after-hours telephonic care, the part-time TX, LA, and TN Licensed Nurse Practitioner will manage high call volumes, perform telehealth visits, and deliver medical support to aging residents across multiple states.

Key responsibilities

• Serve as the first line of support for residents and facility staff after-hours, providing medical care and direction over the phone
• Utilize Curana's telephonic platform to coordinate care between facilities, hospitals, and clinics
• Conduct comprehensive assessments and document encounters accurately in the EMR, ensuring compliance with CMS requirements

Required qualifications

• Master's Degree as a Nurse Practitioner
• Active, unrestricted licensure in Texas, Louisiana, and/or Tennessee
• Nurse Practitioner national certification as ANP, FNP, or GNP
• 3+ years of experience as a Nurse Practitioner
• Background in acute and chronic disease management
