Job Summary
To improve medication adherence rates among patients, the full-time TX Licensed Clinical Pharmacist will lead HEDIS quality measure initiatives, conduct medication therapy management, and analyze data to enhance health outcomes in a remote setting.
Key responsibilities
- Lead efforts to enhance HEDIS medication adherence and other pharmacy quality measures
- Conduct medication therapy management sessions to address barriers and educate patients
- Monitor HEDIS performance metrics and generate reports to assess intervention effectiveness
Required qualifications
- Doctor of Pharmacy (PharmD) degree from an accredited school of pharmacy
- Active pharmacist license in TX or other specified states
- Minimum of 2 years of experience in HEDIS-related medication adherence work
- Strong knowledge of HEDIS measures related to medication adherence
- Proficiency in using electronic health records (EHR) and data analysis tools
