Job Summary
To support patients with chronic conditions, the part-time UT TX Licensed Remote Care Coordinator will develop and implement individualized care plans, monitor patient health data remotely, and educate patients on disease management while working from home.
Key responsibilities
- Develop and support individualized care plans for patients with chronic conditions
- Monitor patient data from remote devices and identify abnormal trends or potential concerns
- Conduct patient assessments and provide education on disease management and program participation
Required qualifications
- Active U.S. Compact RN license required
- Minimum of 1 year of nursing experience in a clinical setting
- Strong clinical assessment and critical thinking skills
- Experience with care coordination and remote monitoring technology preferred
- Proficiency with EHR systems and healthcare documentation
