Job Summary

A company is looking for a Utah License Patient Monitoring LPN or LVN.

Key Responsibilities:

• Outreach to prospective patients and serve as the point of contact for patients enrolled in the RPM365 platform
• Help patients connect to the RPM 365 app and set up their devices
• Engage regularly with assigned patients, monitor their data, and educate them through shared information

Required Qualifications:

• Possesses a Nursing (LPN, LVN, RN) or Clinical Certification (Social Worker, SW assistant, Dietician, etc.)
• Knowledgeable about chronic health conditions and the operation of devices used for monitoring
• Proficient with technology, comfortable with computer platforms and smart phone functionality
• Experienced with client interaction and comfortable with telephone outreach and engagement
• Highly organized, efficient, and exhibits a strong sense of ambition and self-discipline
