Utah Licensed Assessment Coordinator

Job Summary

Providing on-call telephonic risk assessments, the PRN Utah Licensed Assessment Coordinator (RN/Therapist) will assist in identifying appropriate levels of care for potential patient admissions while coordinating the referral and intake process remotely.

Key responsibilities

• Conduct telephonic risk assessments to aid in patient admission decisions
• Coordinate the referral and intake process, maintaining communication with referral sources and families
• Collaborate with the Business Office and UR Department regarding patient financial data and insurance verification

Required qualifications

• Valid Utah clinical or nursing license (CSW, LCSW, ACMHC, LCMHC, RN)
• Preferred one year of experience in a psychiatric inpatient setting with assessment and referral coordination
• Passion for helping others rediscover hope
• Must be at least 21 years of age and able to pass a background check and drug screen
