Job Summary

A company is looking for a Clinical Pharmacist to support market operations through patient-centered clinical consultations and provider engagement.

Key Responsibilities:

• Conduct telephonic patient outreach and provider-facing activities to improve STARs measures
• Analyze STARs and HEDIS reporting to provide insights and best practices to provider groups
• Perform annual medication reviews and develop plans to enhance medication adherence

Required Qualifications:

• Doctor of Pharmacy degree or equivalent experience
• Active, unrestricted pharmacy license in the state of primary residence
• 5+ years of clinical pharmacy practice experience
• 2+ years of experience with Medicare STARs and HEDIS measures
• 2+ years of experience in provider-facing interactions and data analysis
