Job Summary

A company is looking for a Clinical Pharmacist (Temp-to-Hire).

Key Responsibilities

• Provide expert medication therapy consultations and optimize patient outcomes
• Review and evaluate patient medication profiles and counseling sessions
• Assist in orienting new team members and facilitate prior authorizations

Required Qualifications

• Doctor of Pharmacy (PharmD) from an accredited college/university
• Licensed Pharmacist in good standing, with multiple state licenses preferred
• Completion of a PGY-1 residency or equivalent experience
• Knowledge of chronic disease states and their management
• Ability to work effectively within a team and engage with diverse patient populations
