Job Summary

To support a growing telehealth practice, the part-time Utah Licensed Family Nurse Practitioner will deliver personalized primary care through virtual visits, focusing on comprehensive assessments, chronic condition management, and patient education while fostering meaningful relationships with patients.

Key responsibilities

• Conduct comprehensive primary care assessments through telehealth visits, including health history and diagnostic follow-up
• Diagnose and manage acute and chronic primary care conditions in a virtual care setting
• Develop and implement personalized care plans based on patient needs and goals

Required qualifications

• Active Family Nurse Practitioner license/certification with prescriptive authority in Utah
• Minimum of 2 years of experience as a Family Nurse Practitioner in primary care or related settings
• Strong clinical skills in diagnosing and managing a variety of conditions across multiple age groups
• Proficiency with electronic health records and telehealth platforms
• Current CPR/BLS certification
