Job Summary

A company is looking for a Nurse Practitioner (Telehealth) to provide exceptional care to senior Medicare patients primarily through telehealth.

Key Responsibilities

• Conduct annual wellness visits and manage high-risk patients via telehealth, with possible in-home visits
• Document medical histories, symptoms, and medication information thoroughly
• Collaborate with the Primary Care team to ensure holistic patient care and provide patient education on preventive care

Required Qualifications, Training, and Education

• Current Utah Nurse Practitioner license; licenses in Montana, Oregon, Idaho, and Washington preferred
• Nurse Practitioner certification from either the ANCC or AANP
• 2+ years of clinical experience as a NP in Primary Care, Family Medicine, Internal Medicine, Gerontology, or Palliative Care
• Current DEA licensure and Prescriptive Authority post-hire
• Active CPR/BLS Certification and a current Utah Driver's License
