Utah Licensed Pharmacy Care Coordinator

Job Summary

Supporting high-quality patient care, the full-time Utah Licensed Pharmacy Care Coordinator will obtain and review post-discharge medical records, navigate multiple electronic medical record systems, and conduct member outreach to update medication lists while working remotely from Draper, Utah.

Key responsibilities:

• Obtain and review medical records related to recent discharges
• Conduct outreach to members and providers to confirm care relationships and follow up on clinical recommendations
• Identify medication discrepancies and prepare detailed reports for pharmacist review

Required qualifications:

• High School Diploma/GED
• Current, clear, and unrestricted Pharmacy Technician licensure in Utah
• Possess a National Certification (PTCB)
• 2+ years of experience in pharmacy roles
• Intermediate proficiency in Microsoft Office suite, including Outlook, Teams, and Excel
