Utah Licensed RN Case Manager

Job Summary

To coordinate and facilitate quality, cost-effective care for ESRD and CKD patients, the full-time Utah Licensed RN Case Manager will work remotely, collaborating with healthcare teams and managing care transitions between patients, families, and providers.

Key responsibilities

• Manage care coordination for patients, ensuring seamless communication among healthcare teams
• Facilitate quality care delivery while minimizing fragmentation in the healthcare system
• Collaborate with community resources and Regional Operations Manager to enhance patient outcomes

Required qualifications

• A.D.N degree from an accredited school of nursing required; B.S.N preferred
• Current Registered Nurse (RN) license in Utah required
• Minimum of five years' experience in clinical nursing required; three years' experience in renal nursing preferred
• Current CPR certification required
• Basic computer skills and proficiency in MS Word and Outlook required
