Value Based Care Specialist

Job Summary

Providing education and support to patients, the full-time remote Value Based Care Specialist will coordinate services related to chronic care management, behavioral health integration, and remote patient monitoring while ensuring adherence to care plans and compliance with healthcare standards.

Key responsibilities:

• Directly interface with patients using technology to support self-management and chronic disease management programs
• Provide education and coordination of care for patients managing chronic illnesses
• Collaborate with healthcare teams to deliver patient-centered care and follow up on patient needs

Required qualifications:

• An MA, CA, RN, NP, LPN, LVN, PA, or CNA with at least 2 years of relevant work experience
• Familiarity with Chronic Care Management, Remote Patient Monitoring, or similar value-based care programs
• 1-2 years of experience and proficiency in using Electronic Health Records (EHR) software
