Job Summary

A company is looking for a Value Based Care Specialist.

Key Responsibilities:

Interface with patients using technology to support chronic disease management programs
Provide education and coordination for patients regarding their chronic illnesses
Collaborate with healthcare teams to ensure comprehensive patient-centered care

Required Qualifications:

LVN/LP or MA degree & certification with at least 2 years of relevant experience
Familiarity with Chronic Care Management, Remote Patient Monitoring, or similar programs
1-2 years of experience using Electronic Health Records (EHR) software
Ability to manage and support patient healthcare needs through various communication methods
Demonstrated ability to maintain accurate documentation and adhere to compliance standards
