Job Summary
To support a dynamic specialty pharmacy team, the part-time Vermont Licensed Ambulatory Pharmacist will provide advanced clinical pharmacy services, ensuring optimal drug use outcomes for complex patient populations while working hybrid with flexible scheduling options.
Key responsibilities
- Assure optimal drug use outcomes in high-risk patient populations through direct patient care and consultation
- Participate as a member of ambulatory care teams across various specialties, providing clinical and medication expertise
- Support pharmacy operations, including prior authorization and medication access, while monitoring medication therapy outcomes
Required qualifications
- Doctor of Pharmacy degree and completion of an ASHP accredited residency program or equivalent experience
- Active Vermont pharmacist license
- Minimum of 3 years of advanced practice experience in ambulatory care or a clinical outpatient setting preferred
- Specialized knowledge of pharmacotherapy relevant to high-risk patient populations
- Experience in providing clinical consultation and support within specialty pharmacy settings
