Vermont Licensed Housing Case Manager

Job Summary

A company is looking for a Permanent Supportive Housing Case Manager.

Key Responsibilities

• Determine eligibility for program services and conduct assessments to create person-centered care plans
• Assist participants with accessing and renewing Medicaid
• Coordinate services with local providers to ensure alignment with participants' needs and priorities

Required Qualifications

• Master's degree in education, human services, health, counseling, or a related field, or a Bachelor's degree with one year of relevant experience
• Strong commitment to housing first principles and trauma-informed care
• Familiarity with housing and homeless systems of care and federal regulations (HUD/Medicaid)
• Experience with community-based health/behavioral health and/or homeless assistance programs is preferred
• Ability to pass background investigations as required for the position
