Job Summary

A company is looking for a Collaborative Care Mental Health Clinician.

Key Responsibilities
  • Provide triage, clinical assessment, and direct diagnosis of mental health disorders
  • Deliver psychoeducation to patients and primary care providers, and coordinate care and case management
  • Monitor a caseload of patients and oversee a clinical registry for tracking outcomes and service utilization
Required Qualifications, Training, and Education
  • Master's degree with licensure in Vermont as LICSW, LCMHC, or LMFT
  • At least 3 years of direct clinical experience in a medical or mental health setting
  • Experience with pediatric and adult populations preferred
  • Training in acute crisis intervention and suicide risk assessment
  • Familiarity with psychotropic medications and experience with co-occurring disorders
