Job Summary

A company is looking for a System Formulary and Medication Use Policy Manager.

Key Responsibilities

• Establish and maintain medication use policies for regulatory compliance
• Collaborate with partner hospitals and Pharmacy teams to harmonize formulary and clinical services
• Optimize clinical and regulatory performance across the health system

Required Qualifications, Training, and Education

• Bachelor's degree from an ACPE accredited School of Pharmacy required
• Doctor of Pharmacy degree preferred
• Licensed Pharmacist in the state of Vermont or New York
• PGY1 and PGY2 residency training or at least three years of health system pharmacy experience required
• Experience in formulary management and medication use policy maintenance preferred
