Job Summary

A company is looking for a Transitions of Care RN - Hybrid.

Key Responsibilities

• Provide advanced professional nursing care through assessment, planning, implementation, and evaluation of patient healthcare needs
• Deliver direct care to patients and families post-discharge using critical thinking skills for effective care transitions
• Collaborate with the healthcare team to ensure quality care, adhering to organizational standards of patient care and nursing practice

Required Qualifications

• State of Vermont Registered Nurse (RN) license required
• 2-3 years of experience in a relevant clinical area
• All external candidates must obtain a Bachelor of Science in Nursing (BSN) as per offer letter terms
• Current internal RNs are encouraged to pursue a BSN
