Job Summary
Specializing in navigating and documenting Veteran-specific benefits and resources, the full-time Care Navigation and Benefits Specialist will provide guidance on complex cases, assist in developing care coordination workflows, and serve as a subject matter expert on VA healthcare and benefits in a remote environment.
Key Responsibilities:
- Assist in developing veteran-specific care coordination workflows and processes
- Serve as a resource regarding VA healthcare and benefits, helping Care Coordinators navigate available resources
- Provide guidance on complex veteran cases and assist with referrals and escalations
Required Qualifications:
- 5+ years of experience as a clinical social worker, care coordinator, or similar field with progressive program management experience
- 1+ years of remote work experience
- Experience working directly with Veterans and/or within the VA system
- Knowledge of VA healthcare, veteran benefits/resources, and behavioral health
- Experience documenting case notes in a care management or electronic health record platform
