Vice President of Value-Based Care

Job Summary

A company is looking for a Vice President of Value-Based Care Operations, Remote (US).

Key Responsibilities

• Lead operational execution of value-based care strategy and oversee care coordination workflows
• Develop and manage performance metrics to identify process improvements and enhance patient engagement
• Drive integration of services into value-based contracts and monitor financial performance under risk-based contracts

Required Qualifications

• Bachelor's or Master's degree, preferably in Healthcare Administration, Public Health, or Business Administration
• 5+ years of operational leadership experience in healthcare, focusing on value-based care models
• Demonstrated success in developing ACO partnerships and value-based care strategies
• Deep understanding of CMS models and value-based reimbursement frameworks
• Experience with provider engagement and care coordination in post-acute care settings
