Job Summary

Key Responsibilities

Provide direct care via telemedicine to patients with advanced chronic lung and heart diseases
Assess, diagnose, and treat adult outpatients with multiple chronic medical conditions
Coordinate care with primary care providers and advocate for patients facing social stressors

Required Qualifications, Training, and Education

Completion of an accredited graduate level (Master's Degree) Nurse Practitioner program
License in the state of Virginia and Pennsylvania
5+ years of experience caring for adult patients with medical illnesses
1+ years of experience with chronic cardiopulmonary conditions
Active national certification through the American Academy of Nurse Practitioners Certification Board (AANPCB)
