Job Summary

A company is looking for a Regional HSP Bereavement Coordinator.

Key Responsibilities

• Assess bereavement services with care center leadership to meet community, patient, and family needs
• Develop individualized patient bereavement care plans and conduct post-death bereavement risk assessments
• Provide support and outreach to grieving individuals and collaborate on bereavement program metrics and improvements

Required Qualifications

• Bachelor's degree in social work, psychology, counseling, or a related field
• One year of experience in family and/or individual counseling with training in grief and bereavement
• Knowledge of grief and bereavement counseling and the needs of terminally ill patients and their caregivers
• Familiarity with general computer use and online HR services
• Self-management of online training and services
