Job Summary

A company is looking for a Manager, Integrated Care Management/RN/LPC/LCSW/LCMHC for DSNP- Remote in Virginia.

Key Responsibilities

• Manage and coordinate the patient care transitions program to ensure clinical and financial outcomes
• Collaborate with multidisciplinary teams across various care settings
• Oversee departmental budget, compliance with regulations, and manage a team of supervisors and care coordinators

Required Qualifications, Training, and Education

• Bachelor's Degree is required
• One of the following licenses is required: RN, LCSW, QMHP, LCMHC, or LPC
• 3 years of case management experience is required
• 1 year of leadership experience is required
• Extensive knowledge of Medicare/DSNP and LTSS Medicaid requirements is preferred
