Job Summary

A company is looking for a HSS Clinical Coordinator.

Key Responsibilities

• Manage level of care reviews for CCC Plus waiver members and newly identified enrollments
• Collaborate with care coordinators and managers to process LOCERI submissions and reports
• Conduct monthly audits and process necessary documentation for nursing facility admissions and discharges

Required Qualifications

• 4-year or higher degree in a health-related field with 2+ years of direct experience, or current unrestricted independent licensure as a Social Worker, or LPN with 5+ years of direct experience
• 1+ years of experience with MS Office, including Word, Excel, and Outlook
