Virginia Licensed Community Health Worker

Job Summary

As a Virginia Licensed Community Health Worker, the remote position will engage with Medicaid members to facilitate access to care, conduct home and social assessments, and provide member education while working part-time in a team-based structure.

Key responsibilities:

• Complete the New Member process, including welcome calls and scheduling health risk assessments
• Support members in accessing healthcare benefits and maintain engagement with their primary physician
• Address social determinants of health and re-establish relationships with members who have had minimal contact

Required qualifications:

• High School Diploma / GED
• 1+ years of experience or knowledge of community resources and values
• Intermediate proficiency in Microsoft Office applications
• Residency in Virginia and willingness to work core business hours
• Access to high-speed internet and a designated quiet workspace at home
