Job Summary

Managing a defined caseload of adult patients, the full-time Virginia Licensed Home Health RN will perform comprehensive in-home assessments, coordinate care across interdisciplinary teams, and deliver evidence-based nursing care primarily from home while serving the local community.

Key responsibilities

• Manage a defined caseload of adult patients receiving skilled home health services
• Perform comprehensive in-home assessments and ongoing evaluation of patient care plans
• Coordinate care among physicians, therapists, social workers, and acute care teams

Required qualifications

• Graduate of an accredited nursing program required
• Bachelor of Science in Nursing required within 5 years of hire
• 1 year of relevant nursing experience
• Licensed to practice as a Registered Nurse in the Commonwealth of Virginia
• Valid Virginia Driver's License and AHA Health Care Provider BLS certification required
