Job Summary

A company is looking for a RN or LVN/LPN Clinical Patient Admissions Coordinator.

Key Responsibilities

• Timely acceptance or decline of patient referrals based on predetermined criteria
• Establish and maintain communication with clients, family members, and referral sources
• Facilitate the collection of necessary information and documentation for referrals

Required Qualifications, Training, and Education

• Licensed RN, LVN, or LPN in the practicing state
• 3-7 years of experience in facility/physician relationships, preferred
• Minimum of one year experience in home care, hospice, or personal care services, preferred
• Prior use of Electronic Medical Records (EMR), preferred
• Experience dealing with various payors in healthcare, e.g., Medicare and Medicaid, preferred
