Key Responsibilities

• Perform comprehensive medication reviews and develop medication action plans
• Drive medication interventions to ensure adherence to evidence-based guidelines
• Collaborate with clinical teams and provide training on medication management

Required Qualifications

• Bachelor's degree in pharmacy or PharmD
• Active pharmacist license in the state of position
• 3+ years of experience as a pharmacist, preferably in managed care or clinical practice
• PGY1 residency in ambulatory care or related clinical field is preferred
• Board certification in pharmacotherapy or related clinical field is desired
