Job Summary

Focusing on managing health outcomes for specific populations, the full-time Virginia Licensed Population Care Nurse will implement evidence-based interventions, coordinate care, and work within interdisciplinary teams to improve wellness and quality performance remotely.

Key responsibilities

• Implement population interventions to enhance health and wellness, including preventative services and chronic disease management
• Coordinate case management and health education initiatives to ensure comprehensive care for individuals
• Collaborate with interdisciplinary teams to assess and deliver patient care at a population level

Required qualifications

• Bachelor's Degree in Nursing is required
• Registered Nurse (RN) License (Compact or Virginia) is required
• Minimum of 2 years of nursing experience is required
• Health plan experience is preferred
