Job Summary

A company is looking for a Registered Nurse (Spanish-speaking).

Key Responsibilities

• Provide telemedicine visits and education to patients with chronic cardiac and respiratory conditions
• Coordinate care with primary care providers and advocate for patients facing social challenges
• Engage with an interdisciplinary team to optimize patient care and promote the program within the community

Required Qualifications, Training, and Education

• Completion of an accredited nursing program with a minimum of an Associate's Degree
• Current or compact unrestricted Nursing license in Virginia or Pennsylvania
• 5+ years of experience caring for adult patients with medical illnesses
• 1+ years of experience with chronic cardiopulmonary conditions
• Reliable high-speed internet connection (at least 25 Mbps)
