Virginia Licensed Transition Care Coordinator

Job Summary

A company is looking for a Transition Care Coordinator/RN/LCSW/LPC/LPN for a remote position in Virginia.

Key Responsibilities

• Collaborate with the Care Coordinator and Interdisciplinary Care Team to facilitate safe transitions
• Assist in managing care for patients during transitions between different care settings
• Engage with facilities and community resources to support patient needs effectively

Required Qualifications, Training, and Education

• Associate's, Bachelor's, or Master's degree in Health & Human Services or related field
• Licensure as RN, LPN, LCSW, or LPC is required
• Minimum of two years of Case Management or Long-Term Care experience
• Experience in Nursing, Social Work, or Human Services field
• One year of experience working with Medicaid or CCC Plus populations in Virginia
