Job Summary

A company is looking for a Transition of Care Registered Nurse.

Key Responsibilities

• Provide transition of care support for members discharged from inpatient acute care settings
• Coordinate care plans with members, families, and providers to ensure safe discharges to home or SNF settings
• Collaborate with interdisciplinary teams to address member needs post-discharge

Qualifications

• Active RN or LPN license with strong clinical experience in medical/physical health
• Proven expertise in Transitions of Care and Discharge Planning
• Strong understanding of community resources, care coordination, and patient advocacy
• Reliable high-speed internet and the ability to work independently from a home office
