Job Summary

A company is looking for a Virtual Care Coordinator.

Key Responsibilities

• Conduct remote patient check-ins and prepare for clinician visits
• Document medical information in the EMR and assist clinicians during virtual visits
• Coordinate care by managing referrals, prior authorizations, and medication refill requests

Required Qualifications

• High school diploma or equivalent
• 2+ years of experience in a virtual care or healthcare support role
• Familiarity with virtual care workflows in a primary care or chronic care setting
• Comfort with remote tools and EMR platforms, preferably Athena
• Knowledge of obesity medicine or chronic care workflows is preferred
