Virtual Care Navigator

Job Summary

Working remotely in a full-time capacity, the Virtual Care Navigator will manage patient interactions, coordinate referrals, and ensure a seamless patient-centered care experience while collaborating with providers and staff.

Key responsibilities

• Collaborate with providers and staff to deliver an exceptional seamless patient-centered care experience
• Manage scheduling for the virtual team, including appointment creation, reminders, and rescheduling as needed
• Process referrals to secondary care, ensuring timely communication and documentation between patients and specialists

Required qualifications

• At least 3 years of applicable experience, preferably in a Primary Care setting
• High school diploma/GED required
• Proficient in Google spreadsheets and/or Excel, database management, and data tracking
• Access to a quiet/private remote work environment with high-speed internet
