A healthcare company is in need of a Virtual Community Health Partner.

Core Responsibilities Include:

• Following a panel of members to coordinate all aspects of their care
• Providing continuous outreach to support building and maintaining member panels for team
• Steering a care team

Must meet the following requirements for consideration:

• Experience with patient navigation
• Experience managing a panel of clients or patients

• You excel at empathy and human interactions
• Phenomenal communicator
