A healthcare technology company has a current position open for a Virtual Community Health Partner in Waterbury.

Candidates will be responsible for the following:

• Supporting a caseload of members who will rely on you as the main point of contact on their virtual care team
• Providing telephonic care coordination services to members

Qualifications Include:

• Experience managing a panel of clients or patients
• Excel at empathy and human interactions, and want to improve the health of individuals and whole communities
• Experience with patient navigation, case or care management, or any kind of direct service provision
• Save a passion for working within the community you are a part of, or have been a part of in the past
• Phenomenal communicator
