Virtual Hospitalist APP
Job Summary
To support innovative healthcare delivery, the full-time Virtual Hospitalist APP will conduct virtual assessments, manage care coordination, and collaborate with clinical teams remotely to provide hospital-level care to patients eligible for in-home hospitalization.
Key Responsibilities
- Conduct virtual assessments and develop multidisciplinary care plans for adult patients with acute and chronic conditions
- Screen incoming referrals and assess patient eligibility for Hospital Alternative Care while leading onboarding workflows
- Serve as a consultative resource for field teams, guiding care plans and optimizing patient flow through collaboration
Required Qualifications
- Master's or Doctoral degree in Nursing or Physician Assistant Studies
- Current unrestricted licensure and national board certification (NP or PA)
- Minimum 5 years of hospitalist or acute care experience
- Experience in virtual care or telehealth preferred
- DEA registration and prescriptive authority
