Job Summary
A non-profit health insurance company is seeking a Virtual LPN Patient Care Coordinator.
Core Responsibilities of this position include:
- Coordinating care and facilitate discharge to an appropriate level of care
- Identifying and referring quality issues to Quality Improvement
- Identifying and referring appropriate members for Care Management
Skills and Requirements Include:
- Current, unrestricted Registered Nurse (RN) Licensure in state(s) of practice
- OR Current, unrestricted Licensed Practical Nurse (LPN) Licensure in state(s) of practice
- Completion of an accredited registered nursing (RN) degree program or License Practical Nurse (LPN)
- Minimum of two (2) years clinical experience
- Basic data entry skills and internet utilization skills
- Working knowledge of Microsoft Word, Excel and Access
