A health insurance company is filling a position for a Virtual Model of Care Nurse Case Manager I.
Core Responsibilities of this position include:
- Monitoring and evaluating effectiveness of the care management plan
- Ensuring member access to services appropriate to their health needs
- Completing CMS required DSNP activities
Skills and Requirements Include:
- 3 or more years of RN clinical experience
- Ability to type 50 wpm with minimal errors
- Ability to obtain multi-state licensure required
- Current, unrestricted RN license in state of residence required
- Knowledge and work experience with MS Office
- Experience with electronic documentation
