Job Summary

A healthcare technology company is filling a position for a Virtual Registered Nurse Care Manager.

Core Responsibilities Include:

• Working telephonically while managing a panel of members to improve their health holistically
• Using an electronic platform and assessment tool for conducting holistic assessments, care planning, and assisting member care goals
• Providing clinical support to our large team of non-clinical team members, bridging care team facilities and resources

Qualifications Include:

• Have a current, unrestricted RN license in the state in which you are seeking employment
• Have 3+ years of experience providing clinical services to a variety of populations
• Have recent outpatient or telephonic case management experience (prefer 1+ year)
• Have experience as an active participant in continuous quality improvement projects within a provider setting
• Demonstrate the ability to affect change, and have been effective in helping a member or patient adapt new habits, or change behaviors
• Can excel operating with change and uncertainty, with a history of meeting or exceeding target
