WA Licensed Case Manager

Job Summary

A company is looking for a Case Manager, Behavioral Health.

Key Responsibilities

• Engage with members facing medical, mental health, and substance use disorders to build supportive relationships
• Conduct holistic health assessments to identify members' needs and barriers to lifestyle changes
• Create and monitor member-centric case management plans in collaboration with members and healthcare providers

Required Qualifications

• Active WA State or Compact License in Nursing, Behavioral Health, or Psychology
• 3 years of experience with direct patient/clinical care post-licensure
• High-speed internet connection of at least 20 Mbps
