Job Summary

A company is looking for a Case Manager, RN.

Key Responsibilities

• Engage with members facing medical, mental health, and substance utilization disorders to build supportive relationships
• Conduct holistic health assessments to identify members' needs and develop personalized case management plans
• Monitor and evaluate progress towards goals, facilitating communication among the healthcare team to ensure effective care delivery

Required Qualifications

• Active WA State or Compact License in Nursing, Behavioral Health, or Psychology
• 3 years of experience with direct patient/clinical care post-licensure
• High-speed internet connection (minimum 20 Mbps)
