Washington Licensed Ambulatory Care Pharmacist

Job Summary

A company is looking for an Ambulatory Care Pharmacist to provide integrated support to patients and physicians for improved medication adherence and quality patient care.

Key Responsibilities

• Provide medication management, including reconciliation, dose optimization, and adherence counseling
• Collaborate with the interdisciplinary care team to develop individualized care plans based on evidence-based guidelines
• Monitor and evaluate medication regimens to ensure therapeutic goals and compliance with clinical criteria

Required Qualifications, Training, and Education

• Doctor of Pharmacy (PharmD) and licensed pharmacist in WA
• Completion of PGY1/PGY2 residency is preferred but not required
• Strong clinical knowledge and expertise in disease state management and treatment guidelines
• Ability to manage processes and maintain focus on legal compliance
• Energetic, highly motivated team player with strong personal and communication skills
