Washington Licensed Ambulatory Care Pharmacist

Job Summary

A company is looking for an Ambulatory Care Pharmacist to provide integrated support to patients and physicians for improved medication adherence and quality of care.

Key Responsibilities

• Provide medication management and counseling within approved protocols
• Collaborate with the care team to develop individualized care plans based on evidence-based guidelines
• Monitor medication regimens to ensure clinical criteria and therapeutic goals are met

Required Qualifications

• Doctor of Pharmacy (PharmD) and a licensed pharmacist in WA
• Completion of PGY1/PGY2 residency preferred but not required
• Strong clinical knowledge and expertise in disease state management
• Ability to manage processes and maintain legal compliance
• Energetic, highly motivated team player with strong personal skills
