Washington Licensed Ambulatory Care Pharmacist

Job Summary

A company is looking for an Ambulatory Care Pharmacist to provide integrated support to patients and physicians to improve medication adherence and promote evidence-based care.

Key Responsibilities

• Provide medication management, including medication reconciliation and adherence counseling
• Collaborate with the interdisciplinary care team to develop individualized care plans and monitor medication regimens
• Participate in quality improvement initiatives and optimize workflows for medication management

Required Qualifications

• Doctor of Pharmacy (PharmD) and licensed pharmacist in WA
• Completion of PGY1/PGY2 residency preferred but not required
• Strong clinical knowledge and expertise in disease state management and treatment guidelines
• Ability to manage processes with a focus on legal compliance and quality
• Energetic, highly motivated team player with strong personal and communication skills
