Job Summary

A company is looking for a WA Licensed Clinician: ASW, LCSW, APCC or LPCC for Behavioral Health in Family Medicine.

Key Responsibilities

• Provide customized care and treatment plans for patients within assigned Care Pods
• Coordinate behavioral health care with primary care providers and monitor patient clinical symptoms
• Document patient progress and treatment recommendations in electronic health records

Required Qualifications, Training, and Education

• Master's Degree in Social Work, Counseling, or related field from an accredited program
• Licensed to practice in Washington as an associate clinician under supervision or as an independent clinician
• 1-5 years of experience as a licensed professional
